Years before the events of canon, three members of the Mutsuki family were murdered at the hands of a ghoul, leaving behind a single survivor, a girl by the name of Tooru. She was left in the care of the CCG where most orphans from ghoul attacks were sent to be taken care of. Once old enough, Tooru decided that she wanted to become an investigator, and so she was sent to the junior academy. At that moment, about to start a new life, she made the request to be allowed to live and be referred to as male and it was granted. Having never felt comfortable living as a female, Tooru decided to adopt his current identity as a man.
Tooru's time at the academy was not all sunshine and roses, however, because he was bullied for being "queer" and many rumours were spread about him killing cats and his own family. Even when he became an investigator and became a Quinx, a surgically created half-human, half-ghoul, he continued to be pushed around and looked down upon by other investigators. However, not one to cause conflict, Tooru bowed his head to it all and remained quietly respectful in his seclusion from the others. He persevered through the bullying, not going back on his decisions to become an investigator or be male. This stubborn determination stayed with him through the years as he grew, because even though he is the weakest link of his squad he doesn't give up, deciding that he would rather become stronger than leave the others.
This doesn't, by any means, suggest that Tooru doesn't have doubts of himself. In fact, he lacks self-confidence and contains a lot of self-contempt over his own weaknesses, constantly berating himself and becoming incredibly frustrated when he isn't able to call upon his ghoul abilities or when he feels like he can't fight to help and protect his squad. He sees himself as a nuisance and burden to the others because he lacks much of the strength that they have, and this discouragement caused by his anxiety sometimes gets to the point that he feels as if he doesn't belong and isn't needed as an investigator.
Tooru is hardly what one would consider a fighter. Even though he is half-ghoul, a species that survives on eating human flesh, he is anemic and afraid of blood to the point that even rare meat makes him nauseous and uncomfortable. His nature can wholly be described as benign, as he is a man easily made nervous, someone who is self-conscious and stutters, stumbling over his words often. He can be considered naive in how kind and honest he is, in how he believes lying is bad and that their squad should get along and help each other. Even though he and Urie don't get along, when he was hit by one of their other squad members Tooru made him apologize because it's the right thing to do, which he has a strong sense of obligation to uphold. He is very protective of his team, but of all of them this sentiment is strongest with Sasaki, who he will jump to defend and get angry on his behalf when he is insulted because he admires and cares for him greatly.
Tooru is often shown as the observant, concerned mediator when there are disagreements within their team. He is a very kind and amicable person, quick to be there when another needs help or reassurances. When Shirazu was promoted to squad leader, he was there to listen to his worries and offer advice, his gentleness a reliable presence that makes him very likeable. Always polite, Tooru can almost be overly apologetic, not ever wanting to cause trouble or upset anyone, let alone those he cares about, and in comparison to the rest of his lazy, rebellious team he is very respectful and listens to orders given to him.
He is well aware of his own lack of strength, of the fact that he is the weakest of the Quinx, has the lowest RC count and is considered barely different from a human, and is unable to use his Kagune so he solely relies on his Quinqes. However, Tooru can still be brave, almost reckless in his self-sacrificing nature, when the situation calls for it. When an innocent life is in danger, no matter what may happen to him if he intervenes, Tooru will throw himself into the fray in hopes to protect others. He is weak-willed when it comes to fighting, but he receives confidence in himself through the faith others, mainly his superior, Sasaki, have in him. He tries very hard to become a skilled and reliable investigator through his deficiencies and through his fear that he'll one day lose control of his ghoul self and become a monster.

HUMAN ABILITIES Tooru, like the rest of the Quinx Squad, has received combat and investigative training from the CCG, though he is quite lacking in these areas and that of his ghoul powers in comparison to the rest of his squad. He thus relies heavily on his Quinque daggers in order to fight but isn't able to use them very well. Unlike ghouls, the Quinx are able to eat regular food and do not need to consume human flesh to live.
GHOUL ABILITIES Being a Quinx, which supposedly grants him some of the abilities only ghouls have, Mutsuki's body is more resilient than a normal human's. However, because his RC level is so low, almost the amount that a human has, he lacks the superior physical capabilities that ghouls and even the others in the Quinx Squad have. He has thus far been unable to activate his Bikaku-type Kagune (the “liquid muscle” weapon made of RC cells that comes from an organ within them), so any abilities that he has concerning it are unknown. His Kakugan, ghoul eye, is permanently activated, making one of his eyes always red with a grey sclera which causes him to have to wear an eye patch to cover it and avoid suspicion. He also only seems to have semi-decent regeneration abilities.
